Communication between our patients, families, and caregivers is critical for high quality care. We prioritize clear and timely communication, ensuring that each party receives all necessary information.  Together, we can build a strong community around our patients so that we can advocate for their desires and needs.

Our care teams include nurses, physicians, home health aides, chaplains, social workers, and volunteers. The medical team is dedicated to providing skilled and compassionate care to allow our patients to live their remaining days to the fullest. Our social workers, chaplains, and volunteers provide emotional and spiritual support and our administrative team handles coordination and logistic support.   

 

At Golden Time Hospice we are committed to clear and timely communication within our teams, with the family, and with any additional medical care providers. Our patient centered approach allows us to have open communication with our patient while maintaining consistent contact with other members of our care team. Nurses regularly check in with the patient and their family to inform and help them better understand the patient’s condition.

 

Care is usually provided in the patient’s residence where loved ones can easily visit and the patient can feel most at ease. Whether this is in the patient’s home or a care facility, our trained staff will visit the patient to provide care.

At Golden Time Hospice, we provide interdisciplinary support and holistic care for our patients at the end of their life. We treat our patients with the highest level of respect and believe all patients should be comfortable, pain-free, and shown respect regardless of their insurance status, race, gender, ethnicity, sexual orientation, or religion. 

 

It is of utmost importance that our patients feel truly cared for. We will do everything we can to ensure the patient’s needs are met, and concerns are addressed at every step of this difficult process.

OUR SERVICES

WHAT IS HOSPICE?

A HOLISTIC APPROACH TO END OF LIFE CARE

Hospice is end of life care that focuses on enabling the highest possible quality of life. The medical team focuses on the alleviation and management of symptoms and pain based on the patient’s desires. Additional team members focus on the emotional and spiritual care of the patient and also help prepare their loved ones for the journey ahead. The level of necessary hospice care is determined by the patient’s doctor or hospice doctor. The four levels of hospice care are as follows:

 

  • Routine Care takes place in the patient’s residence and consists of routine visits to ensure comfort and provide education.

 

  • Continuous Care takes place in the patient’s residence when it becomes medically necessary to have at least 8 hours of time-intensive care from a nurse to manage pain and medical symptoms.

  • Inpatient Care takes place in a hospital or care facility when 24/7 monitoring from medical staff is necessary.

​​

  • Respite Care takes place at an inpatient hospice facility and is meant to give the patient’s primary caregiver temporary relief from their caregiving duties.
